2009-10 Player Registration & Parent Obligations

Player Sign-Up Information

Walk-in registration has come and gone...but it's not too late to sign your player up. Please follow the instructions below and mail your completed forms & check to:

NUHA has a new process for Registration, please read the instructions below.

STEP 1 - Go to usahockey.com

STEP 2 - Register your NUHA player(s)
- click on "Membership"
- then "Register Online" in the drop down menu
- You will pay $40 Fee online per player ($30 to USA Hockey + $10 to MN Hockey)

STEP 3 - Print and bring the confirmation page and mail in with your registration.

Please mail the following:

1. Registration Form (see form below)

2. Consent to Treat (see form below)

3. USA Hockey Registration Confirmation Print Out (or just add confirmatioin number to registration form).

4. Certified copy* of your child’s birth certificate (if not already on file) - a photocopy is preferred.

5. A check for the appropriate amount made out to NUHA

* Get a certified copy of a birth certificate from the Brown County Recorder’s Office (phone 233-6657). Any Minnesota birth certificate can be obtained from any courthouse in Minnesota for $16. You may keep the actual certified copy and provide NUHA with a photocopy. USA Hockey will NOT accept the complementary copy of a birth certificate provided by a hospital.

Other Registration Notes:

If you have unfulfilled hours of obligation from the 2008-2009 season, please be prepared to pay for them when you register ($20/hour).

There will be areas set up for: clothing sales, registration, new members, hours sign-up & equipment turn-in/check-out.

The United Way offers scholarships to those who qualify. Please see any Board member or contact the United Way directly for an application.

 

New Member Meeting:

If you are a new family to the New Ulm Hockey Association, please mark your calendar for October 6th, 7:00pm at the New Ulm Civic Center, Upper Level Conference Room. Come and meet the Board members, review the NUHA Policy Manual, discuss fundraising and hours of obligation and have your questions answered. See you there!

NUHA Obligations:

Each NUHA family is required to work 32* hours at various fundraisers plus NUHA reserves an additional 8 hours for an extra fundraiser, if needed.

One NUHA family cannot donate hours worked to another NUHA family without prior Board approval.

Hours of obligation not fulfilled prior to next season will be billed at $20/hour.

No one under the age of 18 can work/earn hours of obligation.

Each NUHA family is required to buy/sell a minimum of 10 tickets at $7 each AND provide a cake for the Sauerkraut Dinner**.

Each NUHA family is required to buy/sell a minimum of 6 tickets at $5 each for the Breakfast.

Each NUHA family is requested to buy/sell 10 Hockey Bucks raffle tickets at $10 each.

Another obligation, depending on the team, includes serving as a minor official at home games and tournaments. This obligation is above and beyond and does not count toward your 32 hours of obligation (16 for families listed below under “*”). Your team’s team representative will provide a minor officials’ schedule for working the home games and tournaments.

* Termite and Girls 8U families will be required to work a maximum of 16 hours, unless they have a child at a higher level. Any family that is new to the Association will work a maximum of 16 hours.

** With the exception of members that have only a Termite or Girls 8U player or members that have a Supermite or 10U in Session 1 only. The season for these players will be completed by the time the Kraut Dinner takes place.
